Anaplastic thyroid carcinoma (ATC), also referred to as undifferentiated carcinoma, is a rare and highly aggressive malignant tumor, constituting approximately 2% to 3% of all thyroid gland neoplasms. Thyroid-specific rearrangements such as RET/PTC and PAX8/PPARγ are rarely found in poorly differentiated or undifferentiated thyroid cancer, indicating that these genetic alterations do not predispose cells to dedifferentiation. The histological differential diagnosis for ATC includes: Metastatic disease to the thyroid (e.g., metastatic clear-cell renal carcinoma). Primary thyroid lymphoma. Primary thyroid sarcoma. Poorly differentiated thyroid carcinoma. Squamous cell thyroid carcinoma. Medullary carcinoma. The recommended treatment for anaplastic thyroid carcinoma typically involves surgery, when feasible, accompanied by chemoradiation either concurrently or sequentially. However, most cases of ATC are unresectable at presentation due to the invasion of cervical structures, and surgery primarily aims to secure the patient’s airway. Anaplastic thyroid carcinoma is generally poorly responsive to any multimodal therapy.
