Acute otitis externa (AOE), commonly called ‘swimmer’s ear,’ is prevalent among individuals across various age groups, including children, adolescents, and adults. AOE is characterized by widespread inflammation of the outer ear canal, often exacerbated by prolonged exposure to moisture and frequently associated with swimming activities. Many AOE patients also experience discharge from the affected ear. Most AOE cases involve infections predominantly caused by bacteria. Pseudomonas aeruginosa and Staphylococcus aureus are the most frequently isolated pathogens, with polymicrobial infections occurring in many cases. While less common, fungal infections involving Aspergillus and Candida species have also been reported. Topical antimicrobial agents are recommended as first-line treatment for mild-to-moderate AOE. This typically involves a topical antibiotic, with or without steroids, administered for seven to ten days. Severe cases may necessitate systemic antibiotics effective against S. aureus and P. aeruginosa. Effective pain relief for mild-to-moderate AOE can be achieved with systemic analgesics such as acetaminophen, nonsteroidal anti-inflammatory drugs (NSAIDs), or oral opioids. Topical steroid preparations may offer some relief but are not recommended as standalone therapy due to inconsistent efficacy in clinical trials. In cases where the clinician’s view of the ear canal is obstructed, an expandable wick may be inserted to reduce canal swelling and facilitate the delivery of topical medications. While practices like aural toileting and wick therapy are commonly employed, their effectiveness has yet to be rigorously evaluated in randomized controlled trials. Conversely, ear candling is ineffective and potentially harmful.
