Molluscum contagiosum, commonly known as water warts, is a harmless skin condition characterized by dome-shaped, round, and pinkish-purple skin lesions referred to as mollusca. Molluscum contagiosum lesions can be transmitted through direct skin-to-skin contact, including sexual contact, or indirectly through items like towels, underclothes, toys, razors, and tattoo supplies. Additionally, autoinoculation can occur when patients inadvertently spread the virus from existing lesions to unaffected skin areas. Transmission is also possible in shared wet environments like swimming pools. The incubation period for this virus can range from two weeks to six months, and it infects explicitly keratinocytes. Skin lesions are confined to the epidermis and do not spread systemically. The virus produces proteins that inhibit the human antiviral immune response, contributing to the persistence of skin lesions. Treatment options for molluscum contagiosum include physical removal of the mollusca, topical medications, and systemic treatments. Oral cimetidine is a safe alternative to physical removal for children who fear pain, except for facial involvement. Physical removal methods typically involve cryotherapy with liquid nitrogen or curettage, but they can be painful and may require local anesthesia. Postoperative scarring is a potential side effect. Topical medications may include podophyllum, potassium hydroxide, salicylic acid, benzoyl peroxide, and tretinoin. Laser treatments such as carbon dioxide or pulsed dye lasers are an option but are not typically the first choice. Minimal scarring can be achieved using topical trichloroacetic acid to treat mollusca.
