Genital warts, or condyloma acuminatum, represent a sexually transmitted infection from the human papillomavirus (HPV). These infections primarily spread through skin-to-skin contact, typically occurring during sexual activity. Notably, approximately 90% of individuals exposed to HPV do not develop genital warts, with only about 10% of infected individuals transmitting the virus. Genital warts are caused explicitly by HPV types 6 and 11, which differ from the viral types responsible for cervical and anal cancer. Several risk factors contribute to the persistence of HPV, including age, smoking, immunosuppression, and concurrent infection with multiple HPV types. Diagnosis of genital warts is commonly visual, although a biopsy may be necessary for confirmation. Smaller warts may sometimes be mistaken for molluscum contagiosum. Genital warts typically exhibit characteristics such as elevation above the skin surface, parakeratosis, and nuclear changes indicative of HPV infections, such as nuclear enlargement with perinuclear clearing. The classification of genital HPV includes two main categories: low-risk benign HPV lesions and high-risk neoplastic HPV lesions. Low-risk strains, particularly types 6 and 11, are associated with genital warts, recurrent respiratory papillomatosis (RRP), and low-grade cervical lesions. Despite the high prevalence of genital warts and RRP caused by these types, they exhibit a low likelihood of malignant potential. A significant proportion of genital warts cases exhibit resistance to treatment and tend to recur, especially with repeated sexual infections or the prolonged incubation period of HPV. Morbidity related to the disease results from pruritus, bleeding, and the psychosocial impact of genital lesions. Conversely, mortality arises from the malignant transformation of genital warts to squamous cell carcinoma. The most prevalent local complications involve disfigurement, with untreated and advanced-stage disease posing a risk of malignant transformation, representing the most concerning complication.
• Genital warts are prevalent in approximately 1% of sexually active adults in the United States.
